By entering my information above and clicking Submit I am authorizing CallADoc to bill the credit card or debit card I provided above, on a monthly basis, for the CallADoc program. I understand that I will be billed the $28 or the then current monthly fee about every 30 days, and will have access to the CallADoc network and benefits until I choose to cancel my membership in the program. The first month’s fee of $28 will be billed immediately after I sign up. I can cancel anytime by calling 1-888-428-1855 or emailing service@mycalladoc.com and I won’t be billed anything further. I understand I will be notified of any price increases and will have the opportunity to cancel if I do not want to continue with the program at that time.

CallADoc is not insurance. CallADoc operates subject to state regulation and may not be available in certain states. CallADoc does not guarantee that a prescription will be written. CallADoc physicians do not prescribe DEA controlled substances, non-therapeutic drugs and certain other drugs which may be harmful because of their potential for abuse.
